Transition from pediatric to adult health care can be scary and confusing when you have a complex or chronic medical condition. The STEP (Staging Transition for Every Patient) Program at Children's of Alabama and UAB is teaming with United Ability to answer your questions, help identify adult specialists, assist with care coordination and locate additional services in the community that fit your unique needs. Learn more about the program by attending the upcoming event.

Scholarships are available. The event will also be live streamed at facebook.com/unitedability. (CEUs will not be available via live stream).

Breakfast and Lunch will be provided. Accessible parking will be available

WHO SHOULD ATTEND?

People ages 14-30 who were born with or developed a disability, and their caregivers
Health care providers who work with people with disabilities in this age group, including OT's, PT's, COTA's, PTA's, physicians, nurses, social workers, and students in these areas
